Adult Health Home Care Manager —
Poughkeepsie - AHHCM-KS (Full Time)
Job Title: Bachelor's Degree.
Must have and maintain a valid New York State driver’s license.
This position requires monthly in-person home and community visits within the lower Hudson Valley Region (Orange, Dutchess, Ulster).
Must have 2-5 yrs. experience in the Human Service Field providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism, substance abuse and/or chronic conditions.
We are seeking an individual who shares our vision of partnering with individuals, families and the community to foster hope, wellness and recovery. The Health Home Care Manager is responsible for coordinating services and supports for individuals with a primary diagnosis of severe mental illness and/or other qualifying disorders in order to help with successful integration into and maintenance in the community. Our goal is to partner with and support adults, 18 and up, in developing and strengthening their competencies in order to navigate various community systems while improving overall well-being of the family system through a strength-based and family-driven perspective. The Core service responsibilities of a Care Manager include, but are not limited to: Comprehensive Care Management, Care Coordination and Health Promotion, Comprehensive Transitional Care and follow-up from inpatient to other settings, Individual and Family Support and referrals to Community and Social Supports.
