Job Description:

The Case Planner provides in-home counseling services to families in need of support in developing skill and competencies that will enable them to keep their children in the home and out of placement. The Case Planner will maintain a caseload of 8-12 families and provide change focused interventions with families to improve parental functioning, address life stressors, and improve overall familial functioning. This is happen over a course of 6 to 12 month length of service and a minimum of 2 face to face contacts with each family per month. Has to have at least 1 late night per week to meet the need of working families.
