What is Youth ACT?
Youth ACT (Assertive Community Treatment) is a home-based program that provides comprehensive support to help youth obtain and maintain stability within their homes and communities.
Who Can Benefit?
Youth ages 10–21 living in Dutchess County who:
• have a diagnosed mental health condition and need extra support
• are at risk of hospitalization
• are returning home from a higher level of care
or
• have not found success with traditional outpatient services
Services Offered
• Assertive Engagement
• Individual and family therapy
• Case management & care coordination
• Medication management
• Crisis intervention
• Skill development (life, social, prevocational)
• Individual Psychoeducation
• Family psychoeducation & peer support
• Linkage to ongoing resources
Program Highlights
• Assertive engagement to foster therapeutic connections
• Emphasis on collaborative partnerships with families and community organizations
• Services provided in home and community minimally one to two times per week
(Additional services per week provided based on youth and family need)
• 24/7 phone crisis coaching, intervention, and planning
• Flexible length of care based on youth and family needs
Cost
There is no cost to families. Services are funded through Medicaid and State Aid.
